If you want to change your name
We will prepare a Change of Name Deed that includes the following important declarations:
- You will abandon the use of your previous name
- You will use your new name at all times from the date of signing the deed
- You will relinquish the use of your previous name in all documents and will use your new name going forward

If you want to change your child’s name
Unlike changing your own name, to formally change the name of a child, all persons with parental responsibility for that child must consent to the change of name. If their consent cannot be obtained, you would need to make an application to the court for a Specific Issue Order.
